The triage nurse will also assist with wound care, etc. Job Description Job Title: Triage Nurse Reports to: Director of Nursing Status: Full-time/Exempt The following responsibilities describe the core duties of the Triage Nurse role at Charlotte Community Health Clinic. Key Responsibilities Provide age-specific patient care by assessing, implementing, evaluating, and documenting patient needs, actions taken, and patient responses relative to nursing care. Share patient information with the provider to facilitate care evaluations; update the medical record; accept direction from the provider; make patient referrals as directed to resolve problems or reach goals; review orders for clarity and appropriateness. Gather information and review past allergic reactions. Administer medications and treatments within scope of practice using approved policies and protocols. Use problem-solving and critical thinking to identify and work through issues. Consider patient/family cultural differences and incorporate into care. Identify patient learning needs; provide education and counsel; document patient understanding. Knowledgeable of growth and development across life stages. Facilitate care in alignment with policies, procedures, and telephone triage protocols in cooperation with the physician. Function in emergency situations and facilitate care for all patients. Promote quality patient care and advocate for the patient. Maintain all appropriate licensure and adhere to HIPAA guidelines; ensure confidentiality of patient results. Ensure test results (in-house and referred) are reported to the provider; oversee pending labs as needed. Able to perform CLIA-waived tests. Other duties as assigned. Minimum Qualifications Able to communicate effectively and interact in a friendly, professional manner with a wide range of staff, physicians, and the public. Plan, prioritize, and complete delegated tasks with attention to detail. Ability to multitask and work under pressure. Strong organizational, problem-solving, and critical-thinking skills. Ability to maintain confidentiality and function as part of a team. Knowledge of accounting procedures and principles; ethical behavior when handling sensitive financial information. High level of accuracy, efficiency, and professional customer service. Experience 1-2 years in a physician practice or community-based health setting preferred. Additional Skills Required Ability to work effectively with diverse populations; ability to work independently and use critical thinking; excellent interpersonal and customer service skills; strong oral and written communication; empathy and respect in addressing patient and family concerns; ongoing self-education to improve professional skills. Ability to complete generic and department-specific skills validation and competency testing. Additional Skills Preferred Proficiency in electronic medical records; Experience with Epic (Preferred). Bilingual English/Spanish is preferred but not required. Education Graduate of an accredited Registered Nurse program. Current RN licensure in North Carolina. May be a compact license that is legally allowed to practice in NC. CPR Certification. Physical Requirements The physical demands represent those required to perform the essential functions of this job: Repetitive movement of hands and fingers – typing and/or writing.
