The Treatment and Triage Nurse provides direct patient care to adults with complex, chronic disease in an Ambulatory ICU Patient Centered Medical Home setting. The role functions as part of a large, integrated care team alongside the Primary Care Provider, Social Work, Nurse Care Management, Behavioral Health and others with the goal of providing whole-person care in the outpatient setting in order to avoid Emergency Room and hospital utilization by supporting our members to overcome their multiple barriers to care and self-management.
