Job Description This is a Triage position with about 75% of the work done by phone. Essential Functions Provides appropriate nursing care, including assessment, triage, and referral, while emphasizing patient comfort and well-being. Coordinates medical care for primary care patients, initiates and follow up diagnostic tests, medical reports, referrals, and emergency care.. Participates in multi-disciplinary team case management. Coordinates primary care clinic services with other providers and agencies. Provides well-documented, accurate and sensitive telephone and in-person drop-in advice, and well as health education and self-care instruction. Hours for this Position: M-F 8:30am-5:00pm Qualifications Current and valid California Registered Nurse license and BLS. Minimum one year experience in outpatient clinical practice. Proficient in Microsoft office suite. Preferred: Advanced working knowledge of outpatient care, triage and clinic flow. Demonstrated ability and sensitivity to provide services to persons who are disabled, homeless substance users, HIV (AIDS) infected, and/or psychologically impaired.
