Summary:

Provide pharmaceutical care to the patients of Doylestown Hospital. These services are in support of the safe, rational and cost-effective use of drugs and related products and services in the institution. Pharmacists verify, compound and dispense medications pursuant to a physician order. Monitor drug therapy for appropriateness of drug, dose, route and duration of therapy. Screens for drug interactions, incompatibilities and contraindications. Recognizes potential adverse drug effects and intervenes as appropriate. Provides drug information to physicians, nurses, allied health professionals and patients. Participates in interdisciplinary team conferences and other patient care activities. Provides care for patients of all age groups.

Responsibilities:

  • Responsible for accurate dispensing of medications. Reviews and interprets physician orders properly. Screens for incompatibilities, interaction and contraindications. Contacts Nursing personnel and/or prescriber regarding any questions on drug orders. Accurately fills drug orders. Accurately enters patient orders into the computerized profile. Questions and rectifies duplication of therapy and duplicate PRN reasons. Accurately prepares IV solutions, TPN's, epidurals and other sterile products using proper aseptic technique. Prepares chemotherapeutic agents in a biologic safety cabinet using appropriate safe handling techniques. Checks medications prior to delivery, checks all medications prepared by technicians.
  • Exercises applicable legal and professional controls on controlled and non-controlled substances maintained and dispensed. Maintains proper electronic and paper documentation as appropriate. Follows departmental procedures for the labeling of all medications including high alert medications and controlled substances. Properly processes expired and unusable controlled substances and other medications documenting in accordance with State and Federal Regulations.
  • Dispenses prescriptions to applicable in-patients, out-patients and employees. Accurately enters new prescriptions or refills into the computer system to generate dispensing labels and properly fills these prescriptions. Properly executes paperwork for all prescriptions as required by Law. Accurately processes verbal communication from prescribers and nursing staff. Provides patient information as appropriate.
  • Effectively participates in clinical services provided by the department. Adequately performs Drug Utilization Evaluations within the scope of Hospital Initiatives. Participates in Patient Care Committees and Workgroups as assigned to optimize overall patient care. Intervenes as needed to avoid potential adverse drug reactions. Provides accurate, clinically relevant drug information to physicians, nurses, and allied health professionals as needed. Provide drug information to patients as required utilizing appropriate methods.
  • Meets administrative and miscellaneous requirements of the department. Participates in Departmental Quality Initiative Programs. Reports daily workload activity to the pharmacy management team when needed. Supervisors technicians and organizes and directs their workflow as needed. Utilizes the proper reordering procedures to notify the pharmacy buyer or administrative assistant of needed medications or supplies. Performs monthly unit inspections for assigned areas and monitors expiration dates. Demonstrates a consistent level of performance. Demonstrates good, professional judgment and competence in performing all duties and tasks. Uses technology and automation appropriately to support patient care. Utilizes, maintains and troubleshoots automated systems as required. Identifies problems and issues with automated equipment and reports them appropriately. Stays competent with System upgrades.

Credentials:

  • Pharmacist (Required)
  • Current licensure in Commonwealth of Pennsylvania, Department of State Bureau of Occupational and Professional Affairs

Education or Equivalent Experience:

  • Bachelor of Arts or Science (Required)
  • Graduate of an Accredited College of Pharmacy with BS or Pharm D. degree in Pharmacy Hospital Pharmacy experience preferred
