Position Description

The Peer Case Manager, with support from the Peer Coordinator, is responsible for professional development, life skills education, and peer training activities.

The Peer Case Manager supports the Peer Coordinator with the successful implementation of the CSP Peer Training program, including facilitation of training sessions, group supervision, and one-on-one supervision sessions with peers.

Additionally, the Peer Case Manager will provide case management, linkage to care services, referrals, and support to clients enrolled in the CSP peer training program for the duration of their training and for the 6 months following graduation from the program. This additional case management service will provide support for clients in pursuit of program completion.

Essential Job Functions

Peer Training Program

  • Work with Peer Coordinator to conduct three (3) Peer Education Training cycles per calendar year with a minimum of six (6) trainees per cycle. Each cycle includes twelve (12) sessions, and may be conducted on consecutive days and/or on a weekly basis.
  • Assist with group-level peer-trainee supervisions to review and/or enhance outreach skills and practice.
  • Work collaboratively to conduct individual-level peer-trainee supervision session to develop and conduct trainees personal care plans aimed at addressing the trainees’ goals.
  • Work in collaboration with the Peer Coordinator and Senior Clinical Director to appropriately tailor existing training curriculum, materials, and work processes to serve peer trainees and keep training up to date.
  • Perform data collection, data entry of services provided, ensuring the accuracy and integrity of all information; participate in process and outcome evaluations assessing the efficacy of services provided.
  • Maintain accurate and well-detailed client/group progress notes and caseload files.

Case Management and Navigation

  • Provide client-centered prevention counseling, including assessment, referrals to testing, navigation, and harm reduction services, with accurate, current, and useful information about supportive services.
  • Assist with referrals and navigation to additional social services, including general medical, insurance, mental health, housing, treatment adherence, and other supportive services.
  • Work with Patient Navigators to link clients to health services, as needed, to engage community members in medical services, and to connect and retain them in care and services.
  • Provide care coordination services through face-to-face interactions, phone calls, and/or case conferences with internal/external service providers.
  • Ensure the maintenance of appropriate, accurate, and clear documentation of all services and client activities in client charts.
  • Input data entry for all required documentation into data entry system upon service completion, into both AIRS and eIcare.

Providing support to Integrated Harm Reduction Peers

  • Support the Peer Coordinator in overseeing Integrated Harm Reduction peers.
  • Work collaboratively in the development and facilitation of Peer Training Institute to ensure all department peers are receiving regular training on relevant topics that will help them provide services in the community. Coordinate, schedule, and track the trainings provided and complete required documentation.
  • Educate community members and staff on peer program services and eligibility requirements. Receive resumes from clients interested in obtaining a peer positions in the department.
  • Assist in the creation and maintenance of comprehensive records of all peer participants in the department, including application paperwork, care/treatment/service plans, demographics, peer performance records, and annual assessments of their work.
  • Support peer vocational choices, assisting them with pursuing employment or education and training that matches their strengths and goals.
  • Participate in case conferences for department peers as needed.
  • Act as an advocate for peers.

Education and Certification

  • Required: Bachelor’s degree (Psychology, Mental Health, Social Work, or related field). Associate degree and/or CASAC certification and 2-3 years of relevant work experience may substitute for Bachelor’s degree.

    • Bilingual English/Spanish a plus.
    • Ability to pay attention to details, with strong organizational skills.
    • Excellent verbal and written communication skills.
    • Experience working with marginalized individuals, such as people who use drugs, people experiencing homelessness, veterans, individuals with mental health care needs, and sex workers.
    • Strong commitment to harm reduction.
    • Commitment to working as a member of a diverse staff and peer team.
    • Knowledgeable about HIV and substance use.
    • Strong knowledge and access to New York City social services system, especially in upper Manhattan and the Bronx.
    • Familiarity with online data entry systems such as AIRS, eSHARE, eIcare preferred.
